Nevada Senior Services Receives USAging’s 2026 The John A. Hartford Foundation Business Innovation Runner-Up Award
LAS VEGAS, Nev. (July 20, 2026) - Today, Nevada Senior Services (NSS) was honored by USAging as a runner-up for The John A. Hartford Foundation (JAHF) 2026 Business Innovation Award, which honors community-based organization–health care entity contracts and partnerships that improve health outcomes and quality of life for older adults, people with disabilities and caregivers. The award was presented at USAging's 51st Annual Conference and Tradeshow in San Diego, CA.
NSS in Las Vegas, NV, was recognized for partnering with the University Medical Center of Clark County to deliver the Hospital 2 Home program, which provides dementia-capable care transitions for older adults and individuals with complex conditions discharged from the University Medical Center or several partner hospitals in the Las Vegas area. Operating under a seven-figure contract and demonstrating a self-sustaining model, NSS targets social connection and isolation at their most acute point: post-hospitalization.
"Too often, the people at greatest risk of poor outcomes after hospitalization are also the most socially isolated,” said Jeffrey Klein, NSS President and CEO. “Hospital 2 Home was created to change that by connecting older adults and their caregivers to personalized support, community resources and dementia-capable care at the moment they need it most. The John A. Hartford Foundation 2026 Business Innovation Award was presented to the Agency on Aging of South Central Connecticut in North Haven, CT. A second runner-up was also recognized: Fairfax Area Agency on Aging in Fairfax, VA.
