  • Leads case management activities within the Hospital 2 Home program, providing advocacy, assessment, and coordinated support to help clients maintain independence.

    Responsibilities

    • Deliver person-centered assessments and care plans.

    • Coordinate care with hospitals, SNFs, and community partners.

    • Maintain accurate documentation and reports.

    • Build strong relationships with families and internal teams.

    Qualifications

    • Bachelor’s in Social Work/Human Services (Master’s preferred).

    • NV Social Work license preferred.

    • 2+ years working with chronic conditions or cognitive decline.

  • Serves as a single point of entry to long-term supports, helping clients navigate benefits, access services, and create person-centered plans.

    Responsibilities

    • Conduct assessments and develop service plans.

    • Educate clients on long-term care options and benefits.

    • Assist with applications, referrals, and resource navigation.

    • Document all interactions in the web-based system.

    Qualifications

    • Active Nevada Social Work license required.

    • Bachelor’s degree; experience with older adults and support groups.

    • Strong communication, organization, and crisis-management skills.

  • Provides non-medical personal care and household support to clients of all ages in their homes.

    Responsibilities

    • Assist with bathing, toileting, hygiene, and mobility.

    • Support housekeeping, errands, and daily activities.

    • Travel to client homes across service areas.

    Qualifications

    • 1+ year senior/disabled care experience.

    • Age 18+, reliable transportation, NV driver’s license, insurance.

    • Ability to obtain CPR/First Aid, TB test, physical, and pass background check.

  • Provides in-home respite and caregiver education following hospital discharge, supporting clients with dementia, chronic conditions, or pandemic-related challenges.

    Responsibilities

    • Deliver non-medical respite and personal care.

    • Provide caregiver coaching and support.

    • Assist with basic household tasks.

    • Travel to clients across Las Vegas, Henderson, and North Las Vegas.

    Qualifications

    • 2+ years caregiving, CNA, Med Tech, or LPN experience preferred.

    • Age 18+, reliable transportation, NV license, insurance.

    • Ability to obtain CPR/First Aid, TB test, physical, and pass background check.

  • Supports Adult Day ReCreation participants through direct care, engagement in activities, and daily living assistance.

    Responsibilities

    • Assist with toileting, hygiene, eating, walking, and mobility.

    • Support activities, crafts, exercise, and cognitive programs.

    • Provide light housekeeping and food service assistance.

    Qualifications

    • Experience with older adults preferred; caregiving reliability required.

    • Ability to lift/transfer 100 lbs. with assistance.

    • Bilingual English/Spanish preferred but not required.

  • Full-time Adult Day ReCreation Aide role supporting participants through personal care and activity engagement in Las Vegas and Henderson.

    Responsibilities

    • Provide client-directed care and ADL support.

    • Assist with activities, crafts, exercise, and cognitive programming.

    • Help with food service, housekeeping, and arrivals/departures.

    Qualifications

    • Professional caregiving experience required; experience with cognitive or physical concerns preferred.

    • Ability to lift/transfer 100 lbs. with assistance.

    • Bilingual English/Spanish preferred.
