On Friday, Oct. 16 from 9:00 AM – 11:00 AM, Nevada Senior Services is hosting a community vaccine clinic at our Adult Day ReCreation Las Vegas location. Please register and fill out an Immunization Consent Form to join us!
Vaccines Offered: Flu, COVID-19, Pneumonia, Shingles, RSV, Tetanus, Hepatitis
How to Register
Please register for this event to secure your spot. Walk-ins will be accepted based on availability.
After completing the Registration Form (click the Register Here button on the bottom of this page), you will receive an Immunization Consent Form by email. Please complete this form before your appointment.
Registering for more than one person? Please complete a separate application for each individual. Each person must have their own completed Registration Form and Immunization Consent Form.
What to Bring
Please bring the following to your appointment:
Photo ID
Insurance card (including Medicare Part D if applicable)
Registration Incentive
Individuals who:
Complete the online Registration Form
Complete the online Immunization Consent Form
Receive their vaccination at the clinic
Will be eligible to WIN a prize after completing registration forms and receiving vaccine(s).
Only one application per person. Please make sure both forms are completed online.
